NEVADA LAW REQUIRES CERTAIN IMMUNIZATIONS (those marked *) for public school entry

— unless exempt due to a medical condition or religious belief. Other listed vaccines are

recommended for middle school students to help ensure good health.

WORRIED ABOUT PAYING FOR VACCINES? Many Nevada healthcare providers offer vaccines

at low or no cost to parents as part of the Vaccines for Children (VFC) program (Younger than 19
